Why teeth go yellow in the first place

There are two different processes at work, and they respond very differently to treatment. Extrinsic staining sits on top of the enamel, built up from years of coffee, red wine, curry, tea, tobacco or red sauces. This is the kind that responds well to whitening because the pigment is sitting on the surface and can be lifted off with a bleaching agent.

Intrinsic yellowing is different. It happens inside the tooth structure, either because enamel has thinned with age and lets more of the yellow dentine underneath show through, or because of old antibiotic exposure, fluorosis, trauma to the tooth, or a dead nerve inside a root-treated tooth. This kind of yellowing often needs more than gel. Whitening can still help, sometimes a lot, but it has a ceiling.

What professional whitening can actually achieve

In-clinic whitening uses a much stronger bleaching gel than anything sold over the counter, activated under controlled conditions so it works faster and more evenly. Most patients see three to eight shades of improvement in a single appointment, sometimes more over a course of two or three sessions. For teeth that are yellowed from years of staining but structurally healthy, this is often enough to get a genuinely bright, natural-looking result.

What it will not do is turn a tooth white if the yellow is coming from deep inside the dentine, or if there are old fillings, crowns or veneers on the front teeth. Whitening gel only works on natural enamel, so any existing dental work stays exactly the colour it was, which can leave a mismatch once the surrounding teeth lighten. This is one of the most common surprises patients have, and it is worth flagging before you start rather than after.

When whitening isn't the full answer

If your teeth are badly yellowed and whitening only gets you partway, or if the cause is intrinsic and unlikely to shift much with gel, veneers become the more predictable option. A thin shell of porcelain or composite is bonded over the front of the tooth, covering the discolouration completely rather than trying to bleach through it.
